Read the following for information about the two-step submission process.

    1.) Fill out the form below with your information. All the fields should be filled out, otherwise your submission may not go through or the request may be rejected.

    2.) After you press the submit button, an email will be sent to both our account and the account you provided in the form. Please reply to that email with any changes necessary to your information and attach the work you would like to have submitted.
